Request a Repeat Prescription.

If you are an existing Safetynet patient and need a repeat of your current medication, you can submit a prescription request using the form below.

Requests are reviewed daily, Monday-Friday, by a Doctor and generally take 3 working days to be created. If you have an urgent prescription request, please contact us directly at (01) 233 6990.

This form is for repeat prescriptions only. Please do not use this form to:

  • Request a new medication or a change to your current medication

  • Ask a medical question or request a consultation

  • Report a medical emergency

If you need urgent medical attention, please call 999/112 or attend your nearest Emergency Department.

Request a Repeat Prescription

This form is only for current patients, we can only prescribe medication that you have been given previously. We can not issue a prescription for a new medication or make changes to your current medication.

In some cases, you will be required to attend a doctor’s appointment if the medication you are on requires monitoring or tests before it is prescribed. Your doctor will let you know if this is the case.

  • The information you provide in this form will be collected and processed by Safetynet Primary Care solely for the purpose of reviewing and fulfilling your repeat prescription request.

    What we collect: Your name, date of birth, phone number, pharmacy details, and information about your medication.

    Why we collect it: To verify your identity, confirm your prescription history, and send your prescription to your chosen pharmacy.

    Who may see it: Your information will only be accessed by Safetynet clinical and administrative staff involved in processing your request. It will not be shared with any third party except your chosen pharmacy for the purpose of dispensing your prescription.

    How long we keep it: Your request will be retained as part of your patient record in line with standard medical record retention requirements.

    Your rights: Under the GDPR and the Irish Data Protection Act 2018, you have the right to access, correct, or request the deletion of your personal data. To exercise these rights, please contact us at info@primarycaresafetynet.ie.

  • Please allow 3 working days for your prescription to arrive at the pharmacy. Requests are monitored daily, Monday to Friday, except on bank holidays.

    Urgent prescription requests will be dealt with differently. Please contact us at (01) 233 6990 to discuss.

    If your request requires a review by a doctor before the prescription can be issued, we will contact you.

    If you have not heard from us after 4 days, please contact us to make sure we have received your request.

  • No. This form is only for repeat prescriptions — medication you are already taking. If you need a new medication or want to change your current treatment, please book a consultation.

  • If you have run out of medication and need it urgently, please contact the clinic directly at (01) 233 6990. If you are experiencing a medical emergency, call 999/112.

  • We only send prescriptions to the chosen pharmacy on the form above. You can’t collect prescriptions from the clinic.

  • Please describe it as best you can (what it's for), and we will check your records. We will need to contact you to confirm the details, and you may need to attend the service.

  • In some cases, particularly for medications that require monitoring (such as blood tests or blood pressure checks), we may need to see you before issuing a repeat prescription.

    Controlled Drugs and any drugs not on your repeat prescription list, i.e., I had betnovate once for my eczema, now I want it again for flare up after 1 year. Will require a GP appointment before the prescription can be given.

    You may also need to attend a doctor’s appointment if you have not been seen by a Safetynet GP in 6 months or more. Patients see the doctor for a medication review every 6 months; for some medications, it may be more frequent.
