Safetynet Primary Care is a medical charity committed to delivering critical healthcare to the most vulnerable, reaching people on the margins of society, those who are the sickest and most in need of care and without access to healthcare. The organisation works from the principle that high quality medical care should be available to all those who need it regardless of market forces and income. The organisation enables a coordinated approach to medical care for patients not accessing mainstream services through management of a web based medical record system.

We are seeking an experienced Project Worker to support the Homeless Primary Care Team (HPCT) and Inclusion Health Hub (IHH) in delivering essential healthcare services to homeless and vulnerable populations.
Key Responsibilities include client care coordination, advocacy, administrative support, and outreach to ensure dignified and professional service delivery. The role involves collaboration with multidisciplinary teams and developing referral pathways.
Requirements include (see job description for more detail) a third-level qualification in social care, psychology, sociology, or related work with vulnerable groups, with at least 2 years' relevant experience; expertise in the homeless sector and working with marginalised groups; strong multitasking, decision-making, scheduling, and problem-solving abilities. Excellent communication and teamwork skills. Fluent English.
