Safetynet Primary Care administer a web based medical record system for patients seen by specialised primary care teams working with homeless people.  The Socrates Practice Management system is in use because many people living in precarious circumstances do not continue to see one mainstream GP. Therefore, it is very important that those who do attend have access to their medical records regardless of which team / practitioner has seen them. The SNPC medical record system enhances patient care by allowing a more complete view of the patents medical history and care by any of the following services who engage with the patient. The purpose of the system is to ensure quality healthcare with a reduced risk of duplication or fragmentation of care.
